Our office requires payment at time of service for all deductibles, co-pays and co-insurances.  If you are concerned about the amount that may be due from you, schedule a consultation with one of our doctors to determine your out-of-pocket expense prior to surgery.

BILLING QUESTIONS??

ATTENTION GENERAL ANESTHESIA PATIENTS!

If you are having general anesthesia during your surgery, the following instructions must be followed or your surgery may be canceled!

• Nothing to eat or drink after midnight the night before your surgery.  This includes water, food, gum, candy, mints, ice, etc.
• No smoking and/or chewing tobaccto prior to surgery.
• You MUST have someone drive you to the appointment who will remain in our waiting room area during your entire surgery.  You may not be dropped off and pikcked up later and you cannot use any form of public transportation!

 CANCELLATION POLICY

Please be advised that our office requires at least 24 hours notice to cancel or reschedule an appointment. 

If you do not give our office24 hours notice, you may be charged a $25.00 fee for missed appointment time which will need to be paid in full before your appointment can be rescheduled.
