Our office requires payment at time of service for all deductibles, co-pays and co-insurances.  If you are concerned about the amount that may be due from you, schedule a consultation with one of our doctors to determine your out-of-pocket expense prior to surgery.

ATTENTION GENERAL ANESTHESIA PATIENTS!

If you are having general anesthesia during your surgery, the following instructions must be followed or your surgery may be canceled!

Nothing to eat or drink after midnight the night before your surgery.  This includes water, food, gum, candy, mints, ice, etc.

No smoking and/or chewing tobacco prior to surgery.

You MUST have someone drive you to the appointment who will remain in our waiting room area during your entire surgery.  You may not be dropped off and picked up later and you cannot use any form of public transportation!
